Care at Home Medicaid Waivers
Care at Home (CAH) waiver programs provide case management for a variety of services, therapies and medical care for children with developmental disabilities and/or complex medical conditions.
CAH waivers are administered by New York State. CAH Waivers I and II are directed by New York State Department of Health. The OPWDD CAH Waiver is directed by the New York State Office for People with Developmental Disabilities. Children ages birth to 18 must meet certain medical criteria for eligibility and admission into the program.
Referrals are made to the Elmcrest Care at Home program from Onondaga and surrounding counties. Family members, human service agencies, medical and educational personnel may refer children to the program by contacting Elmcrest Disability Services at (315) 492-2131.
