Community Habilitation

An individualized service, Community Habilitation promotes independence and integration into the community for youth with disabilities. Youth and families are encouraged to direct the path of this program with the support of Elmcrest Community Integration Specialists.

Youth engage in skill-based activities in the community including:

  • Peer relationship building
  • Community safety
  • Daily living activities
  • Social awareness
  • Literacy
  • Communication
  • Financial management
  • Exploration of their community

Participation in Community Habilitation for youth requires eligibility from the New York State Office for People with Developmental Disabilities (NYS OPWDD) and enrollment in the Home and Community Based Medicaid Waiver.

Referrals to Elmcrest are made through Medicaid Service Coordinators (MSC).
